Phyllis J. Beswick Forth was born September 8, 1921, daughter of Clarence W. and Maggie M. Kennedy Beswick. She married Leland S. Forth September 27, 1940, in Clinton, Iowa.
Surviving are daughters, Beverly (John) Lunquist and Janice (Larry) Wiersema; sons, Clarence (Norma) and Neal (Melisa) Forth; grandchildren, Eric (Lisa) Wiersema, Gregg (Jackie) Wiersema, James (Michelle) Forth, John (Brenda) Forth, Paul (Lynnette) Forth, Michael (Jessica) Forth, Mmaggie Forth, Melanie (Zach Schutte) Love and Tyler Ortmeier; great-grandchildren, Hailey and Matthew Wiersema, Ashley Forth, Jareth (Roxane) Kuberski, Jonathan ad Kayli Kuberski, Rhett and Jace Forth, Jaden Forth, Briauna Love ad Jazzyn and Skyla Schutte; three great-great-grandchildren, Neveab, Isaac and Lily Kuberski and brother, Dewitt 'Bud' Beswick and sister-in-law, Bernita Forth.
She was preceded in death by her parents; husband August 1, 2000; former daughter-in-law, Gaylene Forth; sister, Adele Berge and brothers, Howard and Tom Beswick.

Widely regarded as one of the finest players ever to appear for Sunderland.
Ambrose Fogarty signed from Sunderland in 1963 and went on to play well over 100 times for the club.
In 1964 he became the first Sunderland player to make an international appearance for Ireland,
